– 15% (dependent on your company size) for each claim.

Employees visit a dentist or health care provider of their
choice to receive treatment. Unlike insurance plans, there
are no limitations or exclusions on specific treatment, other
than the dollar amount you’ve chosen.
There
are no pre-authorizations or other time consuming paperwork
to be filled out. Claims can be submitted electronically,
by fax or by mail, dependent on the provider. This product
is available for reimbursement of dental or healthcare related
